Internal Shoulder Rotation (ISR)—the rapid medial rotation of the upper arm bone (humerus) toward the midline of the body—is the single largest biological generator of racket head velocity in the modern tennis serve and overhead, and plays a vital role in modern topspin groundstrokes.
Anatomy & Kinesiology of Internal Shoulder Rotation
Internal Shoulder Rotation (medial rotation) is the movement of the anterior surface of the humerus turning medially toward the midline of the body within the glenohumeral joint. It is driven by four primary muscle engines:
- Subscapularis: The largest and strongest muscle of the rotator cuff, originating on the subscapular fossa and inserting on the lesser tubercle of the humerus.
- Pectoralis Major: Massive anterior chest muscle providing powerful horizontal adduction and internal rotation.
- Latissimus Dorsi: Broad posterior back muscle pulling the humerus downward and internally.
- Teres Major: Synergist muscle assisting the latissimus dorsi in adduction and medial rotation.
The Scapulohumeral Rhythm & Plane of the Scapula
For high-velocity ISR to occur without impingement, the scapula must upwardly rotate and posteriorly tilt along the thoracic cage. Striking athletes must perform rotational swings within the "scapular plane" (approximately 30° to 45° anterior to the coronal plane), ensuring optimal congruence between the humeral head and glenoid fossa.
